DOXEPIN

It is a tricyclic antidepressant being a highly selective 5-HT uptake inhibitor. Have a quicker onset of antidepressant action, causing lesser cardiovascular complications. This drug has an edge over other tricyclic compound for being prefered in older age group. It has lesser anticholinergic side effects.

Indications

Psychoneurotic & psychotic depression, anxiety with depression, atypical depression.


Dosage

75-100 mg daily in 3-4 div. doses. Severe cases : Initially 50 mg trice daily increasing upto 100 mg thrice daily if reqd. Single daily dose : upto 150 mg.


Contra-Indications

It is not recommended for children below 12 yrs. Epilepsy. Urine retention with enlarged prostate. Hepatic impairment.


Special Precautions

Pregnancy, lactation. Suicidal tendencies, may potentiate action of other C.N.S. depressants. Treatment with anticholinergics or sympathomimetics, concurrent electro convulsive therapy. Glaucoma. Paediatrics: Not recommended below 12years. Pregnancy: Use with caution. Lactation: Safety not established. Elderly: The dose to be adjusted according to tolerance and requirements.


Side Effects

Impaired alertness, nervousness, tremor, insomnia, orthostatic hypotension, fatigue, ataxia, epileptiform seizures, jaundice. Change in libido, impotence, gynaecomastia, galactorrhoea.


Drug Interactions

Reduces hypotensive action of clonidine, reserpine, gaunethidine, methyldopa and bethanidine. Alcohol enhance sedative effect.
