A Mother, an Uncle, and Two Tiny Lives: Rare Liver Donations Save Filipino Twin Brothers
Sometimes, the greatest medical miracles begin with the greatest acts of love. For 23-month-old Filipino twin brothers Tyler and Kelly, survival became possible because their mother and maternal uncle each donated a part of their liver, allowing doctors at Indraprastha Apollo Hospitals, New Delhi, to perform two rare life-saving liver transplants.
The landmark surgeries have created history at the hospital, becoming the first paediatric twin liver transplant among the 645 paediatric liver transplants performed at Apollo Delhi so far.
The complex back-to-back procedures were led by Dr. Anupam Sibal, Group Medical Director and Senior Paediatric Gastroenterologist, and Dr. Neerav Goyal, Senior Consultant and Head of Liver Transplant Surgery.
The twins were born prematurely, weighing only 2 kg and 2.4 kg. Their birth had brought joy to the family after the heartbreaking loss of another infant. But within just two weeks, both babies developed persistent jaundice and pale stools. Investigations revealed Choledochal Cyst Type IVA, a rare birth defect in which the bile ducts become abnormally enlarged, leading to progressive liver damage and, eventually, liver failure.
As months passed, the boys' condition worsened. They suffered repeated internal bleeding, fluid build-up in the abdomen, poor growth and frequent hospital admissions. Despite every possible treatment, doctors concluded that liver transplantation was the only way to save both children.
Finding two living donors for twin brothers needing transplants almost at the same time was an enormous challenge. Their mother immediately stepped forward and donated a portion of her liver to one son. When their father was found medically unfit to donate, the twins' maternal uncle did not hesitate to volunteer. His liver donation made it possible to save the second child. While his wife, brother-in-law and sons recovered from surgery, the father became the family's pillar of strength, caring for all four of them.
Calling the case one of the rarest in paediatric transplantation, Dr. Anupam Sibal said liver transplants in twins are uncommon, and it is even rarer for both siblings to require transplantation simultaneously. He said this was the first such case among Apollo's 645 paediatric liver transplants. He added that the hospital has pioneered several complex procedures over the years, including combined liver-kidney transplants, transplants in babies weighing just 3.5 kg and ABO-incompatible liver transplants. Describing the twins' remarkable journey, he said they were "Born Together, Fought Together and Saved Together."
Dr. Neerav Goyal said Tyler's surgery lasted more than 15 hours, while Kelly's transplant took over 13 hours. Both operations involved highly complex reconstruction of blood vessels and bile ducts. Today, both boys have healthy liver function and are recovering well, giving their family hope for a normal childhood.
Mr. Shivakumar Pattabhiraman, Managing Director of Indraprastha Apollo Hospitals, said the family's decision to travel from the Philippines to India reflected their faith in Apollo's transplant programme. He said the successful outcome was made possible by the dedication of a multidisciplinary team committed to delivering world-class care.
Lt. Gen. Dr. Bipin Puri said the case demonstrates what modern medicine can achieve when advanced technology, surgical expertise and teamwork come together.
He added that such achievements continue to strengthen India's reputation as a global destination for complex liver transplants.
Doctors also urged parents not to ignore persistent jaundice or pale stools in newborns, as these could be early warning signs of serious liver disease.
Early diagnosis and timely referral to specialised paediatric liver transplant centres can dramatically improve a child's chances of survival and a healthy future.
