Doctors Say Muscle May Matter More Than Fat in Heart Failure Cases
For years, doctors across the world have struggled to explain a strange and uncomfortable medical mystery. Why do some heavier patients with heart failure often survive longer than thinner ones?
Now, scientists believe they may finally have an answer — and it could completely change how millions of patients think about their bodies, ageing, and survival.
New research presented at the 2026 Heart Failure Congress of the Heart Failure Association of the European Society of Cardiology suggests the real protective factor may not be excess fat at all. It may be muscle.
The findings come from an analysis of 1,467 patients enrolled in the Scottish BIOSTAT-CHF study, one of Europe’s major heart failure research projects. Researchers found that patients who looked relatively thin overall but carried excess fat around the waist faced some of the worst outcomes.
Scientists described this dangerous body pattern as the “lemon-on-sticks” phenotype — a person with a narrow frame but a swollen middle. Behind the unusual nickname lies a grim warning.
The problem may not simply be fat. The real danger could be weakness.
Patients in this category showed stronger signs of fluid congestion, higher levels of inflammation, and elevated amounts of a heart stress marker called N-terminal pro-B-type natriuretic peptide, or NT-proBNP — a substance released when the heart is struggling to pump blood properly.
Most alarming was the survival data.
According to the analysis, these patients had more than double the risk of death from any cause compared to patients who had higher body weight but less abdominal fat. Researchers adjusted the results for age and sex, strengthening confidence in the findings.
“Obesity is a risk factor for cardiovascular disease, yet paradoxically, in patients with established heart failure, a higher body mass index is associated with better survival,” said Doctor Camilla van der Hoef from University Medical Centre Groningen in the Netherlands while presenting the findings.
But she explained that body mass index, or BMI, tells only part of the story.
BMI measures total body weight relative to height, but it cannot distinguish whether that weight comes from fat, retained fluid, or muscle. That limitation has frustrated heart specialists for years.
Inside laboratories and hospital clinics, researchers have increasingly begun tracking body composition instead of weight alone. Waist-to-hip ratio measurements, blood biomarkers, inflammation profiles, and muscle loss patterns are now drawing growing attention.
The new study appears to strengthen that shift.
Patients with high BMI but lower waist-to-hip ratios — meaning less dangerous belly fat concentration — actually showed better outcomes. Meanwhile, people with lower body weight but central fat accumulation appeared biologically frailer despite looking slimmer.
In simple terms, a person may appear thin outside while their body is quietly losing protective muscle and becoming metabolically weaker.
That changes the conversation completely.
For decades, many patients with heart failure were left confused by the so-called “obesity paradox”. Families often struggled to understand why sudden weight loss in elderly heart patients sometimes coincided with worsening health instead of improvement.
Researchers now suspect muscle preservation may be one hidden explanation.
Muscle does far more than help people walk or lift objects. It supports metabolism, stores energy during illness, stabilises blood sugar, and may help the body tolerate severe stress. When muscle wasting begins — especially in chronic diseases like heart failure — the body becomes far more vulnerable.
“Our analyses suggest that BMI and waist-to-hip ratio capture distinct biological and clinical phenotypes,” Doctor van der Hoef said. “Further longitudinal studies of changes in body composition over time are warranted.”
The findings arrive at a time when heart failure cases are rising globally due to ageing populations, diabetes, obesity, and high blood pressure.
For millions living with weakened hearts, the message emerging from this research is surprisingly human. Survival may not depend only on becoming lighter. It may depend on staying stronger.
And in the silent battle against heart failure, muscle could be the body’s hidden shield.
