Electrical Nerve Therapy Shows Promise for Fibromyalgia in Real-World Study
A widely used but often overlooked therapy is gaining renewed attention after a new “real-world” study found it can significantly ease the pain and fatigue associated with fibromyalgia.
The treatment, known as transcutaneous electrical nerve stimulation—or TENS—has long been available as a noninvasive option for pain relief. Now, researchers say it may offer meaningful benefits even when added to standard care.
In the study, published March 27 in JAMA Network Open, patients using TENS alongside physical therapy reported greater reductions in pain and tiredness compared to those relying on physical therapy alone. The findings are especially notable because all participants were already receiving conventional treatments, including medications and rehabilitation.
“The study shows that TENS provides an added benefit on top of any relief from other treatments,” said lead author Dana Dailey, an assistant research scientist at the University of Iowa.
TENS works by delivering mild electrical currents through the skin using adhesive electrodes, typically placed on areas such as the back. These currents are thought to interfere with pain signals or alter how the body perceives discomfort.
While previous research has explored TENS in tightly controlled clinical settings, this latest trial marks its first large-scale test in everyday medical practice. The study followed 384 patients across 28 outpatient clinics in the United States, offering a clearer picture of how the therapy performs outside the lab.
Participants were split into two groups: one received both TENS and physical therapy, while the other continued with physical therapy alone. Those using TENS were instructed to apply the treatment for about two hours daily over six months, either in one session or spread throughout the day.
The results emerged quickly. Within just two months, patients in the TENS group reported noticeable reductions in pain—both at rest and during movement. In contrast, those receiving only physical therapy did not see the same improvements in movement-related pain, a key challenge for people with fibromyalgia.
Researchers also found that consistency mattered. Patients who stuck closely to the daily regimen experienced the greatest relief.
In a telling development, participants who initially received only physical therapy were later given access to TENS—and soon reported similar improvements. That crossover result, researchers say, strengthens the case for the therapy’s effectiveness.
Beyond pain relief, another benefit stood out: reduced fatigue. For many living with fibromyalgia, persistent exhaustion can be just as debilitating as pain, yet treatment options remain limited.
“We were excited to see that patients also had less fatigue,” said senior author Kathleen Sluka, a professor of physical therapy and rehabilitation science. “Right now, there are no good treatments for fatigue, so seeing any improvement there is powerful.”
By the end of the six-month study, about 80% of patients using TENS said they found it helpful—and just as many were still using it, suggesting strong acceptance and adherence.
For researchers, perhaps the most striking takeaway is that the therapy held up under real-world conditions, where variables are harder to control and results often fall short of clinical trials.
“Often, when you move a randomized, controlled trial into a real-world setting, it doesn’t work,” Sluka noted. “But this intervention still works.”
As interest grows in non-drug approaches to chronic pain, TENS may be poised for a broader role—offering patients a relatively simple, at-home option to better manage the daily burden of fibromyalgia.
