Eyes Could Hold the Key to Predicting Heart Disease and Aging Risk, Study Finds

Your eyes may reveal much more than just your vision health — they could also offer clues about your heart and how fast your body is aging. A new international study published in the journal Science Advances has found that the tiny blood vessels in the retina, located at the back of the eye, can help predict a person’s risk of heart disease and biological aging.

For decades, doctors have known that the eyes can reflect systemic health conditions. Through retinal scans, ophthalmologists can detect signs of hypertension, diabetes, high cholesterol, and even early heart disease. But this new research goes a step further — suggesting that changes in the eye’s microvasculature could indicate not only cardiovascular risk but also how quickly a person is aging at the cellular level.

A Window to the Body’s Microvascular Health

The study analyzed retinal scans, genetic data, and blood samples from more than 74,000 participants across four major international studies, including the Canadian Longitudinal Study on Aging (CLSA), the U.K. Biobank, and others.

“We chose to study the eye, and specifically the retina, because it provides a unique, non-invasive view of the body’s microvasculature — the network of small blood vessels that reflect overall vascular health,” explained Dr. Marie Pigeyre, associate professor at McMaster University, Canada, and senior author of the study.

According to Dr. Pigeyre, advances in artificial intelligence (AI) have made it possible to use retinal imaging to detect subtle vascular changes linked to inflammation, aging, and heart disease — often before symptoms appear. “Traditional risk factors like smoking, blood pressure, and cholesterol do not fully capture a person’s cardiovascular risk. Retinal imaging adds another powerful dimension to our ability to predict and prevent disease,” she added.

Simpler Retinal Blood Vessels, Higher Heart Risk

The research team found that participants with simpler and less branched retinal blood vessels were more likely to develop cardiovascular disease and showed markers of faster biological aging, including inflammation and shorter lifespan.

“The finding that less complex retinal blood vessels are linked to heart disease and reduced life expectancy highlights the retina’s potential as an early, noninvasive biomarker,” said Dr. Pigeyre. “It can help identify at-risk individuals and enable personalized prevention strategies.”

The study also identified two key proteins — MMP12 and IgG–Fc receptor IIb — that may drive these changes by influencing inflammation and tissue remodeling. These findings, researchers say, could pave the way for new therapies aimed at slowing vascular aging.

Ophthalmologists See Promise in Early Detection

Commenting on the study, Dr. Benjamin Bert, a board-certified ophthalmologist at MemorialCare Orange Coast Medical Center, USA, said the results demonstrate the powerful diagnostic potential of retinal imaging.

“The retinal microvasculature is the only microvasculature we can observe noninvasively — yet it mirrors the same vascular network found in the brain and kidneys,” Dr. Bert explained. “This is why eye exams have long been used to identify underlying conditions like diabetes and hypertension. Studies like this one could help us predict disease risk even earlier, using just a photograph of the eye.”

Dr. Bert also highlighted how AI is revolutionizing retinal diagnostics, noting that previous studies have used AI to determine age, sex, and even the risk of dementia from retinal images. “These noninvasive tests could soon become a routine tool for monitoring our overall health,” he said.

Hope for Preventive Medicine — But More Work Ahead

Cardiologist Dr. Craig Basman, Associate Director of the Structural & Congenital Heart Program at Hackensack University Medical Center, said the study offers both “apprehension and optimism.”

“The research suggests that individuals with less complex retinal vessel networks may be genetically predisposed to a higher risk of heart disease, stroke, and shorter lifespan,” Dr. Basman noted. “However, the exciting part is that the eye offers a painless, noninvasive way to assess vascular health — potentially providing early warnings before symptoms emerge.”

Dr. Basman cautioned that more clinical validation is needed before such retinal scans can be used as a standard diagnostic tool. “This study opens an exciting new avenue in preventive medicine. By looking into our eyes, we may one day be able to see and address the risks to our heart and longevity much earlier than ever before,” he said.

A Glimpse into the Future of Health Screening

As the line between ophthalmology and cardiology continues to blur, scientists are optimistic that retinal imaging could soon play a key role in routine health screening. The eye — long celebrated as the “window to the soul” — may, in fact, be the most accessible window into the body’s overall health and aging process.
