Global Cholera Cases Near Half a Million in 2025: WHO

By September 2025, mankind will find itself confronted with a frightening reality: cholera, which was previously in decline, is returning at alarming rates. Between January 1 and August 31, 2025, the World Health Organisation (WHO) reported 462,890 illnesses and 5,869 deaths in 32 countries. The Eastern Mediterranean region bore the greatest burden, followed by Africa, Southeast Asia, the Americas, and the Western Pacific. Europe remains the only region unaffected by outbreaks. 

August this year alone saw 57,215 new cases and 758 deaths, down 16% and 22% from July. However, experts caution that this dip provides little relief. In August, the global stockpile of oral cholera vaccine (OCV) was only 2.6 million doses, less than half of the 5 million threshold considered needed for emergencies. 

Cholera is a centuries-old disease, yet its continuation in the twenty-first century demonstrates serious global imbalances. Cholera is caused by Vibrio cholerae and spreads through contaminated water and food. If left untreated, it can kill within hours. However, it is fully avoidable with effective water, sanitation, and hygiene (WASH) systems. 

Today, old systems are collapsing under the strain of modern challenges. Extreme weather events such as floods, droughts, and cyclones are impacting water supply and sanitation in fragile states. Families who have been displaced to overcrowded camps in conflict zones are especially vulnerable. 

"Poverty and instability have always fuelled cholera. What is new is the added pressure of climate change," said a health worker in South Sudan, one of the worst affected countries this year. The country, like Sudan, is experiencing its highest case count this century. Meanwhile, the Comoros experienced its first epidemic in over 15 years, a harsh reminder that no community is immune. 

Behind the statistics is a horrific human toll. One in every five occurrences involves a youngster under the age of five, whose smaller bodies are more susceptible to dehydration. Parents in Yemen, Somalia, and Ethiopia report that they rush ill children to clinics hours away, only to find medicine and rehydration supplies running scarce. 

In remote places, delayed treatment is often lethal. The rising incidence of community deaths, according to WHO, demonstrates how vulnerable health systems are straining to keep up. 

Vaccination programmes remain an important tool for epidemic control, but global supplies are under severe strain. In 2022, WHO implemented a single-dose plan to stretch limited supplies, exchanging long-term protection for larger coverage. The release of a new vaccine, Euvichol-S, in 2024 has helped replenish stocks, but demand continues to outpace supply. 

This shortfall forces frontline health staff to make difficult decisions about who will receive protection and who will have to wait. The implications are most severe in camps and settlements, where rapid spread can overwhelm clinics within days. 

Cholera's comeback on such a large scale is particularly devastating given decades of progress. In 2024 alone, 60 countries reported instances, an increase from 45 in 2023. The WHO has described the current worldwide danger as "very high," emphasising how delicate the struggle has become. 

The Ending Cholera Roadmap 2030 aims to cut mortality by 90% by improving WASH systems, providing prompt access to treatment, and coordinating multisectoral responses. However, reaching that aim will necessitate significant funding in addition to emergency vaccine efforts. 

Cholera epidemics follow the rains and tides, reflecting the world's larger health disparities. Experts warn that unless clean water and climate resilience are prioritised, families will continue to lose loved ones to diseases that the world already knows how to combat.
