Implant That Stimulates Vagus Nerve Shows Hope for Patients With Severe, Treatment-Resistant Depression

A small device implanted in the chest that delivers gentle electrical pulses to a major nerve connecting the brain and body may offer new hope to people living with severe, treatment-resistant depression, according to a new large-scale study.


Researchers report that more than one in five patients who received the implant were virtually free of depressive symptoms two years after treatment—an outcome rarely seen in people whose depression has failed to respond to multiple medications.

The findings, published on January 13 in the International Journal of Neuropsychopharmacology, focus on vagus nerve stimulation (VNS), a therapy that targets the left vagus nerve, a key communication pathway between the brain and internal organs. The nerve plays a role in mood regulation, stress response and emotional processing.

“We were shocked that one in five patients was effectively without depressive symptoms at the end of two years,” said Dr. Charles Conway, lead author of the study and Director of the Treatment Resistant Mood Disorders Center at Washington University in St. Louis. “Seeing results like that for this complicated illness makes me optimistic about the future of this treatment.”

Studying the sickest patients

The study enrolled nearly 500 patients with moderate to severe major depressive disorder across 84 clinical sites in the United States. All participants had depression that had not responded to at least four different antidepressant treatments, placing them among the most difficult-to-treat patients. Nearly three-quarters were so severely affected that they were unable to work.

Each patient received the VNS Therapy System, an implantable device manufactured by LivaNova USA Inc., which also funded the trial. To allow for meaningful comparison, only half of the devices were activated during the first year.

The results were striking. Among patients whose devices were turned on from the start, 69% experienced a meaningful response within one year, defined as at least a 30% reduction in depressive symptoms. Importantly, the benefits were not short-lived.

After two years, more than 80% of those who responded initially maintained or even improved their gains across measures of depression severity, quality of life and daily functioning.

“These results are highly atypical,” Dr. Conway said, noting that most studies involving treatment-resistant depression show poor long-term outcomes. “We’re seeing people getting better and staying better.”

Life-changing improvements

Even partial improvement can have a profound impact on people with severe depression, the researchers stressed. A 30% reduction in symptoms may mean the difference between being unable to function and regaining a degree of independence.

“For someone whose illness has left them paralyzed by life, that kind of improvement can be transformative,” Conway said.

Among patients who showed a substantial response—defined as a 50% or greater reduction in symptoms—92% were still doing better after two years, highlighting the durability of the treatment.

Regulatory and cost hurdles remain

Vagus nerve stimulation is not new. The U.S. Food and Drug Administration (FDA) has already approved the therapy for both epilepsy and depression. However, its use has been limited, largely because of cost. The implant and associated care remain out of reach for many patients.

The researchers hope the new evidence will prompt the U.S. Centers for Medicare and Medicaid Services (CMS) to reconsider coverage for the therapy. If CMS approves reimbursement, private insurers are likely to follow, significantly expanding access.

“We believe this trial represents the sickest treatment-resistant depressed patient population ever studied,” Conway said. “There is a dire need to find effective treatments for these individuals, who often have no other options.”

A lasting option for a chronic illness

Depression that does not respond to standard treatments is often chronic, disabling and associated with a high risk of suicide. In that context, even sustained partial improvement can dramatically alter a patient’s life trajectory.

“With vagus nerve stimulation, we’re seeing not just improvement—but improvement that lasts,” Conway said. “For patients with few alternatives left, that is extraordinarily meaningful.”

While further policy decisions will determine how widely this therapy becomes available, the study adds to growing evidence that neuromodulation approaches like VNS may play an increasingly important role in the future of mental health care.
