Is Your Breath Raising Your Blood Pressure? How a Hidden Brain Signal Is Giving New Hope to Millions
High blood pressure, also known as the "silent killer," affects more than a billion people worldwide and is a major cause of heart attacks and strokes. For millions of people, the illness persists despite healthy food, frequent exercise, and medicine, leaving them angry and, in many cases, mistakenly blaming themselves. Scientists now believe that the condition may be caused by a misfiring signal deep within the brain that governs breathing, rather than by lifestyle.
Researchers discovered a previously unknown link between how we breathe and how tightly our blood arteries are regulated. The discovery, published this week in Circulation Research, one of the world's most respected cardiovascular journals, suggests that a small area in the brainstem—the most ancient part of the brain that controls automatic functions like breathing and heart rate—is a key driver of high blood pressure.
"The lateral parafacial region is recruited into action, causing us to exhale during a laugh, exercise, or coughing," says lead researcher Professor Julian Paton, Director of Manaaki Manawa, the University of Auckland's Centre for Heart Research. "These exhalations are what we call 'forced' and driven by our powerful abdominal muscles."
In everyday breathing, no such effort is required. As Paton observes, "A normal exhalation does not need these muscles to contract; it happens because the lungs are elastic." Scientists were surprised to discover that the same breathing-control area connects to nerves that tighten blood vessels, causing blood pressure to rise.
"We discovered a new area of the brain that is causing high blood pressure. " Yes, the brain is to blame for hypertension!" Paton declares. When researchers turned off this region in a laboratory, blood pressure returned to normal.
This finding provides an essential "aha" moment for patients who have done everything correctly but are still unable to control their blood pressure. The study implies that the cause could be an excessive breathing signal rather than poor habits. It also provides a practical hint: persons who rely heavily on abdominal muscles to breathe, even at rest, may be unintentionally causing frequent blood pressure increases.
The consequences are especially important for people who suffer from sleep apnoea, a prevalent condition in which their breathing repeatedly stops while sleeping. Carotid bodies, tiny sensors in the neck that check oxygen levels, become extremely active in these patients, sending distress messages to the brain. The same carotid bodies, the researchers discovered, trigger the affected brain region.
"Targeting the brain with drugs is tricky because they act on the entire brain and not a selected region such as the parafacial nucleus," according to Paton. The breakthrough occurred when scientists recognised they didn't need to treat the brain directly at all.
"Our goal is to target the carotid bodies, and we are importing a new drug that is being repurposed by us to quench carotid body activity and inactivate 'remotely' the lateral parafacial region safely," he continued, "without needing to use a drug that penetrates the brain."
If effective in human trials, this method could pave the way for a new, non-invasive treatment for those with treatment-resistant hypertension, particularly those suffering from sleep apnoea. Doctors may be able to stabilise blood pressure without affecting brain chemistry or requiring surgery by calming a hyperactive signal in the neck, a breakthrough that could improve care for millions of people suffering from the silent killer.
