Mexican Researchers Propose New Guide for Often Overlooked Dementia Conditions
Researchers at the University of New Mexico (UNM) have developed a novel model for characterising vascular dementia, a prevalent type of cognitive impairment caused by a disease in the brain's small blood vessels that has traditionally gotten less study attention than Alzheimer's disease.
The study, conducted by Dr Elaine Bearer, a pathology professor at the UNM School of Medicine, appeared in the American Journal of Pathology. The study attempts to fill a substantial vacuum in the literature by thoroughly describing the many vascular diseases that contribute to dementia.
Vascular dementia is frequently associated with illnesses such as hypertension, atherosclerosis, and diabetes. These disorders can damage microscopic blood arteries in the brain, resulting in oxygen shortage, inflammation, and small, often undiagnosed strokes that destroy neural tissue over time. However, the precise illness processes have not been fully classified, making diagnosis and treatment development difficult.
In this preclinical study, Dr Bearer discovered ten separate disease pathways that contribute to vascular brain damage. To investigate them, her team examined post-mortem human brain tissue from a storehouse of donations from deceased New Mexicans with dementia. Using specialised microscopic techniques and stains to show damaged vasculature, they discovered an important finding: a significant number of patients previously diagnosed with Alzheimer's disease also had considerable vascular pathology.
"We suspect that in New Mexico maybe half of our Alzheimer's people also have vascular disease," Dr Bearer told me.
The proposed framework describes ways for detecting and scoring the severity of these vascular alterations. The study is funded by the UNM Alzheimer's Disease Research Centre (ADRC), which recently obtained a five-year, $21.7 million grant from the National Institutes of Health (NIH). Dr Bearer worked on this project with Dr Gary Rosenberg, a neurologist and ADRC director who has long investigated the link between vascular disease and dementia symptoms.
The study also addresses an emerging source of concern: the presence of nano- and microplastics in brain tissue. According to Dr Bearer's preliminary findings, these particles are more prevalent in the brains of dementia patients and are associated with higher levels of inflammation.
"Nanoplastics in the brain represent a new player in the field of brain pathology," Bearer said, saying that the health consequences are yet unknown and that all current models of dementia may need to be changed in light of this finding.
The greater significance of this work stems from its ability to develop a standardised system for categorising vascular dementia. The NIH has discussed organising a consensus group with neuropathologists from other universities to develop formal standards. Such a method is regarded as a key first step towards more precise diagnosis and targeted treatment research for a disorder that affects millions globally.
