New Eye Drops Offer Reading Freedom for Age-Related Vision Loss: Study
Millions of adults over the age of 40 are constantly reaching for reading glasses just to read a menu or a text message. A new study suggests that a pair of prescription eye drops could provide a straightforward, non-surgical solution for addressing this widespread ailment, known as presbyopia.
Presbyopia is the slow loss of the eye's capacity to concentrate on surrounding objects, which is a natural part of ageing and affects almost everyone at some point. The latest study, presented by Dr Julia Benozzi, looked into a combination of two well-known medications: pilocarpine, which shrinks the pupil to improve focus, and diclofenac, an anti-inflammatory that helps prevent potential side effects such as eye irritation.
The study included 766 patients, with a mean age of 55. They were divided into three groups and given eye drops with varying concentrations of pilocarpine (1%, 2%, or 3%), as well as a fixed dose of diclofenac. The patients used the drops twice a day. The researchers then tested their ability to read a typical eye chart for reading, known as a Jaeger chart, without spectacles.
The findings were significant. "Our most significant result showed rapid and sustained improvements in near vision for all three concentrations," observed Dr Benozzi. "One hour after having the first drops, patients had an average improvement of 3.45 Jaeger lines."
The improvements were significant and long-lasting. 99% of patients in the group treated with 1% pilocarpine solution achieved excellent near vision. Higher amounts also produced excellent outcomes, with many patients able to read three or more additional lines on the chart. This improvement was maintained for a median of 434 days, or approximately 15 months.
The treatment was also well tolerated. 32% of patients reported momentary impaired eyesight as the most prevalent side effect. A small proportion of people reported mild discomfort or headaches. No significant adverse effects were reported, and no patients discontinued their medication.
Dr Benozzi emphasised that treatment can be individualised to each individual. "Our study revealed that optimal pilocarpine concentrations could be individualised," she added, noting that those with milder presbyopia responded best to the 1% solution, while others with more advanced visual loss required higher levels.
She concluded: "These findings indicate that this combination medication is a safe, effective, and well-tolerated alternative to standard presbyopia management... Eye care clinicians now have an evidence-based pharmaceutical option that extends presbyopia therapy beyond glasses and surgery."
However, an independent expert urges cautious optimism. Professor Burkhard Dick, who was not engaged in the research, pointed out the study's flaws. "The single-centre retrospective study by Dr. Benozzi suggests that eye drops may improve near vision for up to two years, but the limited design means the results may not apply to everyone." He also mentioned that using these components for a long time might, in rare cases, cause problems like poor night vision or retinal detachment with pilocarpine, and corneal issues with extended use of NSAIDs. "Broader, long-term, multi-centre studies are needed," he said.
Dr Benozzi intends to conduct additional studies to assess quality-of-life improvements and determine exactly how the drops function. For now, this intriguing treatment represents a possible new road to being free of reading glasses, albeit it is not designed to replace surgery for everyone.
