New Study Maps Eight Distinct Trajectories of Long COVID, Reveals Why Recovery Varies Widely
A major new study published in Nature Communications has identified a range of distinct recovery patterns among people suffering from long COVID, offering fresh insight into why some patients improve while others continue to struggle months after infection.
The research, led by Dr. Tanayott Thaweethai of Massachusetts General Hospital Biostatistics, analyzed data from 3,659 adults enrolled in the National Institutes of Health’s RECOVER cohort. Participants were evaluated repeatedly over a period of three to 15 months after contracting COVID-19, using a long COVID research index designed to measure persistent symptoms.
The team discovered eight unique symptom trajectories, underscoring the complexity and variability of long COVID. According to the findings, 5 percent of individuals experienced a consistently high burden of symptoms throughout the study period.
Another 12 percent showed a fluctuating but nonresolving pattern, with symptoms that rose and fell over time. Meanwhile, 14 percent initially showed no signs of long COVID at the three-month mark but reported worsening symptoms by month 15, suggesting delayed onset in a significant subgroup.
Among participants who met the long COVID threshold at three months—about 10 percent of the cohort—the outcomes varied significantly. Nearly 46 percent continued to experience persistent, heavy symptom burden, 35 percent had moderate symptoms, and 19 percent appeared to recover over time.
Dr. Thaweethai said the findings could play an important role in shaping the next phase of long COVID research. “The variability we identified will enable future studies to evaluate risk factors and biomarkers that could explain why patients vary in time of recovery, and help identify potential therapeutic targets,” he noted.
The study highlights ongoing efforts to better understand what has become one of the most complex and prolonged aftereffects of the COVID-19 pandemic. The authors noted that several team members have ties to the biopharmaceutical industry, reflecting the growing involvement of multiple scientific sectors in tackling long COVID.
