Nitrous Oxide Shows Rapid Relief for Severe Depression

A large meta-analysis performed by the University of Birmingham has found new evidence that nitrous oxide, a gas frequently used for pain management, may give fast-acting advantages for individuals suffering from major depressive disorder (MDD), including those who have not improved despite taking numerous antidepressants. The findings, published in eBioMedicine, provide new impetus to the global hunt for rapid-response medicines that can assist patients who continue to struggle despite normal therapy. 

MDD is a chronic mood illness characterised by intense melancholy, low energy, and a lack of interest in everyday activities. A related disorder, treatment-resistant depression (TRD), arises when a patient does not improve despite taking two different antidepressants. TRD affects nearly half of UK patients, according to researchers, and comparable or even higher percentages are anticipated in many regions of the Global South, where long wait times, a lack of mental-health professionals, and irregular access to medicines can all contribute to poor outcomes. 

The new analysis examined seven clinical trials and four protocol papers from teams around the world. All investigated whether nitrous oxide (N₂O) may safely alleviate depressed symptoms. Nitrous oxide is commonly used in hospitals for short operations since it relaxes patients and relieves discomfort. Scientists believe it may also alleviate depression by binding to glutamate receptors in the brain. Glutamate is a chemical messenger that helps regulate mood, and disturbances in its signalling have been associated with severe depression. The gas appears to function similarly to ketamine, another fast-acting antidepressant. 

The researchers discovered considerable variations between single and repeated dosages. In three of the experiments, a single inhalation of 50% nitrous oxide resulted in "rapid and significant reductions" in depressed symptoms within 24 hours. However, this improvement subsided after a week. In contrast, "repeated dosing over several weeks led to more durable improvements," implying that continuous treatment may be required for the effects to last. 

Kiranpreet Gill, the study's first author, emphasised the magnitude of the challenge: "Depression is a severe illness, compounded by the fact that antidepressants have no substantial effect for nearly half of all people diagnosed with depression. There is a growing corpus of research on using treatments from different clinical domains to address low mood. This study puts together the best available evidence to show that nitrous oxide has the potential to deliver rapid and clinically meaningful short-term benefits in people with severe depression." 

She went on to say that nitrous oxide "could form part of a new generation of rapid-acting treatments," and she emphasised the importance of further research into how to safely employ repeated dosages. 

So far, safety data appears favourable. Some patients suffered temporary nausea, dizziness, or headaches, but they resolved quickly, and no trial revealed short-term safety issues. Researchers highlighted, however, that much more research is needed to understand the long-term implications. 

Professor Steven Marwaha, the senior author, described the findings as a significant milestone for individuals who have been unsuccessful with current treatments. He emphasised the critical need for options that supplement conventional care paths. 

The study aligns with ongoing research at the Mental Health Mission Midlands Translational Centre and the Birmingham Clinic for Advanced Mood Disorder Management (CALM), which both aim to provide evidence-based treatments for diverse and underprivileged communities—especially important in the Global South, where economic challenges often make it hard to access mentalhealth services. 

The team is preparing the first NHS trial in the UK to determine if nitrous oxide can be administered safely and effectively to treat serious depression. These discoveries may potentially help to increase treatment options for millions of people around the world, particularly in areas where long wait times and substandard care have left many people without viable options.
