Popular weight-loss drugs can heal damaged hearts: Research
Scientists have discovered that medicines used for obesity can force tiny, blocked blood vessels to open up after a heart attack, preventing permanent tissue damage.
Popular weight-loss medications may have an unexpected, lifesaving advantage for heart attack victims. A recent study by researchers at the University of Bristol and University College London reveals that medications often taken for obesity and type 2 diabetes can protect the heart from serious damage soon after a heart attack by correcting a vital "plumbing" defect inside the heart muscle.
The study published in Nature Communications shows that GLP-1-mimicking medications, which are commonly used to control blood sugar and help with weight loss, can reopen small blood vessels that often get blocked even after emergency treatment.
A heart attack blocks a major artery that supplies blood to the heart. Doctors act fast to reopen this important artery using stents or clot-busting medications. However, over half of patients' blood still does not reach some areas of the heart muscle. This circumstance occurs because very small blood arteries deep within the heart are forcibly squeezed shut. Doctors refer to this problem as "no-reflow.". When blood flow is disrupted, heart tissue becomes depleted of oxygen and begins to die, increasing the chance of heart failure within a year.
Dr Svetlana Mastitskaya, Senior Lecturer in Cardiovascular Regenerative Medicine at Bristol Medical School, noted that "in nearly half of all heart attack patients, tiny blood vessels within the heart muscle remain narrowed, even after the main artery is cleared during emergency medical treatment." This causes a condition known as 'no-reflow', in which blood is unable to reach specific areas of the heart tissue.
"Our earlier research has demonstrated that blood artery constriction contributes considerably to 'no-reflow', a condition that raises the chance of mortality or hospitalisation for heart failure within a year of a heart attack. However, our most recent findings are startling, as we discovered that GLP-1 medications may prevent this condition.
To further understand how this works, researchers looked at animal models. They focused on pericytes, which are tiny contractile cells. These cells can wrap around small blood arteries and contract in a manner similar to a fist. When the heart is deprived of oxygen, known as ischaemia, these cells constrict, narrowing the arteries. The researchers observed that GLP-1 medicines activate potassium channels, which are microscopic gateways in cells that help them relax. When these channels open, the pericytes relax, allowing blood vessels to expand and re-establish oxygen flow.
Professor David Attwell, Jodrell Chair in Physiology "With an increasing number of similar GLP-1 drugs now being used in clinical practice for conditions ranging from type 2 diabetes and obesity to kidney disease, our findings point to the opportunity for these existing drugs to be repurposed to treat the risk of 'no-reflow' in heart attack patients, offering a potentially life-saving solution."
The relevance is immediate. Millions of people worldwide are currently using GLP-1 medications safely. Previous research has demonstrated that they minimise major cardiac events, regardless of weight loss. Because some medications have already been approved, doctors may not need to spend decades developing a new treatment. Clinical trials could look into their emergency use in heart attack units much sooner.
For nations such as India, where heart disease is still the leading cause of death, the potential of using an existing medicine to prevent lifelong heart damage provides new hope. If subsequent human trials corroborate these findings, emergency rooms may soon be using weight-loss medications not only to trim waistlines but also to preserve heart muscle and lives.
