Prediabetes Is Rising Fast in Overweight Children, Even Without Extra Weight Gain, Study Warns

More and more overweight kids are getting prediabetes at a young age, and the number is rising faster than doctors thought it would, even if the kids aren't becoming much bigger. A big long-term study from Finland has prompted new worries that metabolic damage may already be happening in children, which could lead to type 2 diabetes later in life. 

When blood sugar levels are higher than normal but not high enough to be declared diabetes, this is called prediabetes. It doesn't always make noise, but it means that the body isn't able to control sugar levels well. If not treated early, it can lead to type 2 diabetes, which is a lifelong disease with major problems. 

The study analysed health data of 602 children aged six to 16 years who received treatment for overweight or obesity during an 18-year duration. Their data were juxtaposed with that of 483 youngsters from the general population who were monitored longitudinally. The results were rather surprising. Over one-third of overweight youngsters had prediabetes, and a small but concerning number already had type 2 diabetes. Prediabetes was far less common among kids in the general community, and diabetes was not detected at all. 

The researchers discovered that prediabetes escalated significantly with advancing age and throughout adolescence, a period characterised by the body's inherent increase in insulin resistance. Insulin is the hormone that helps sugar get from the blood to cells. When insulin production is hampered, sugar builds up in the blood. 

The trend over time was one of the most worrying things that came out of the study. In the early 2000s, a little more than 11% of overweight kids had prediabetes. That number rose to 50 per cent by 2017–19. This rise happened even though there weren't any big changes in body weight, body mass index, or other usual risk factors like blood pressure or cholesterol levels. 

The increase, on the other hand, seemed to be directly linked to two things: fatty liver disease and changes in the health of the mother. Kids with metabolic dysfunction-associated steatotic liver disease, which is often known as fatty liver, were far more likely to have prediabetes. Fatty liver happens when too much fat builds up in the liver and makes it hard for it to do its typical jobs, like controlling blood sugar. 

The study also found that the number of mothers who were overweight before or during pregnancy went up a lot. During the study period, maternal overweight increased from 20 per cent to approximately 69 per cent. Not all children of overweight mothers acquired prediabetes; however, the pattern indicates an increasing impact of early-life and pregnancy-related factors on children's long-term metabolic health. 

This discovery is very relevant to India. Today, many young women in Indian cities have to deal with excessive hours of work, not enough exercise, and diets high in calories. A 34-year-old working mother in Delhi who had trouble gaining weight while pregnant may now have her 12-year-old child flagged for high blood sugar at a regular health check, even though the child doesn't look very overweight. Paediatricians all around India are hearing more and more of these kinds of stories. 

The authors of the study emphasised that obesity alone does not fully account for the increase in prediabetes. Deteriorating quality of the diet, lack of physical activity, the way fat is distributed in the body, and early metabolic programming may all play a role in the problem. They also warned that fatty liver disease can happen even if a kid doesn't look very overweight, which is an important sign that people often ignore.

Children who get prediabetes are more likely to get type 2 diabetes earlier than their peers. This condition in turn can affect the heart, kidneys, and eyes. India, already renowned as the diabetes capital of the world, could have an even bigger problem with the disease if early metabolic concerns in children are not found and treated. 

The authors say that overweight kids should have their blood sugar and liver function checked earlier, and they want families to start making changes to their lifestyles before pregnancy. They warn that the results are from a Finnish community, but the trends are probably applicable to many nations dealing with an increase in childhood obesity and maternal overweight. 

The message is plain, in short. Being overweight as a youngster is no longer the only symptom of trouble. Children may be developing hidden sugar problems much earlier than predicted, even if they don't gain weight. Recognising and acting on these early signs could be the key to stopping the next wave of diabetes.
