Silent Bone Loss May Be Killing Thousands of Postmenopausal Women Before Fractures Even Happen, Study Warns

For years, osteoporosis has been treated mainly as a bone disease that raises the risk of fractures in older women. But a major new study now suggests the condition may be far more dangerous than previously believed. Researchers have found that postmenopausal women with osteoporosis may face a sharply higher risk of death itself— even before a devastating fracture changes their lives forever.

The findings, published in The Menopause Society's journal Menopause, showed that osteoporosis was linked to a 47% increased risk of mortality in postmenopausal women, especially when bone mineral density in the femur—the long bone in the thigh—dropped within a critical range.

The study analysed nearly 3,000 postmenopausal women using dual-energy X-ray absorptiometry, commonly known as a bone density scan. The scan measures how strong or weak bones are by using very low-dose X-rays. Researchers found that women whose femoral bone density fell between 0.46 and 0.71 g/cm² faced significantly greater mortality risks.

Doctors say the findings expose a frightening reality about a disease often ignored until it is too late.

In crowded orthopaedic wards and elderly care units across the world, many women arrive only after a sudden hip fracture or spinal collapse. Then comes surgery, prolonged bed rest, infections, loss of mobility, and, for some, an early death.

Many never fully recover.

According to global estimates cited in the study, osteoporosis affected nearly 19.7% of the world’s population in 2022. Women were disproportionately affected, with prevalence reaching 23.1%. Researchers project that by 2030, around 263 million people worldwide could be living with osteoporosis, including nearly 154 million women.

Health experts say menopause plays a central role.

As women age and oestrogen levels fall during menopause, the body begins losing bone faster than it can rebuild it. Bones gradually become porous, brittle, and dangerously weak. The femur, spine and hips are especially vulnerable. The damage usually develops silently over years without obvious symptoms.

Many women discover the disease only after a fall that changes everything.

Previous research has already shown that older women face a significantly higher risk of death within one year after suffering hip or spinal fractures. However, the new study goes further by suggesting that low bone density itself may act as a warning sign of broader decline inside the body.

Researchers believe bone loss may reflect deeper ageing processes affecting muscles, balance, blood vessels and overall metabolic health.

The study, titled “Femoral bone mineral density and mortality risk in postmenopausal women: a National Health and Nutrition Examination Survey cohort study", found a strong inverse relationship between bone density and mortality. In simple terms, stronger bones appeared linked to better survival chances.

“Osteoporosis often remains a silent threat after menopause, despite its profound effect on women’s lives—from loss of height, poor balance, and reduced mobility to disfigurement, pain, and even premature death,” said Dr Monica Christmas.

“Getting checked early and taking steps to prevent problems, like eating foods high in calcium, exercising regularly, and using hormone therapy when needed, can greatly help bone health and lower the chances of fractures, heart disease, some cancers, and dementia.” It’s time we bring this conversation to the forefront,” she added.

Researchers and public health experts say the warning carries particular importance for countries like India, where large numbers of women enter menopause with poor nutrition, low calcium intake and limited awareness about bone health. Vitamin D deficiency, reduced physical activity and a delayed diagnosis further worsen the crisis.

In many clinics, bone density scans remain underused despite being one of the most effective tools for early detection.

Doctors say prevention is still the strongest defence. Regular walking, sunlight exposure, strength-building exercises, milk products, pulses, green vegetables, and timely medical advice can help preserve bone strength. In high-risk women, hormone therapy and medicines may also slow bone loss.

The new findings are now forcing experts to rethink osteoporosis not merely as a bone disorder but as a serious marker of ageing and survival itself. The danger for millions of postmenopausal women quietly living with weakening bones may extend beyond fractures alone.
