PROBENECID

PROBENECID

Uric acid is largely reabsorbed by active transport, while less of it is secreted. Probenecid, therefore, promotes its excretion and reduces its blood level. It is neither analgesic nor anti-inflammatory.

Indications

Chronic gout. Secondary hyper uricaemia due to drugs or disease.


Dosage

Initially 0.25 g twice daily, gradually increased to Max. 1 gm twice daily. Not ecommended for children.


Contra-Indications

Blood dyscrasias. Renal uric acid stones. Therapy initiation during acute attack.


Special Precautions

Peptic ulcer. Pregnancy, renal impairment. Maintain adequate fluid intake. Paediatrics: Not recommended for children under 2 years. Pregnancy: Use with caution. Lactation: Use with caution. Elderly: Reduced dose may be necessary.


Side Effects

Dispepsia, rashes, convulsions. Toxic dose may causerespiratory failure.


Drug Interactions

Thiazides, aspirin, alcohol reduce its efficacy. Enhances effects of oral antidiabetics, indomethacin, sulfonamides, acyclovir, methotrexate.
