GRANISETRON

Granisetron is a serotonin (5-HT3) receptor antagonist used primarily as an antiemetic medication. It works by blocking the action of serotonin in the central nervous system and the gastrointestinal tract, which helps prevent nausea and vomiting.

Indications

Nausea and vomiting induced by cytotoxic chemotherapy and radiotherapy.


Dosage

Parenteral : Adult: 10mcg/kg intravenously within 30 minutes before initiation of chemotherapy. This should be given only on the days of chemotherapy. Children: 2-16 yrs of age : 10mcg/kg. Oral: Adult: 2mg once daily (upto 1 hour before chemotherapy) or 1mg twice daily (first 1mg to be given upto one hour before chemotherapy and the second 1mg tablet to be given 12 hrs. after the first dose) The drug is to be administered only on the days of chemotherapy. Children: Not recommended.


Contra-Indications

Hypersensitivity. Children below 2 years.


Special Precautions

-Cardiovascular Monitoring: Granisetron may cause QT interval prolongation, leading to potential cardiac issues such as arrhythmias. Monitor patients for signs of these complications. -Liver and Kidney Impairment: Use with caution in patients with liver or kidney problems, as granisetron is metabolized and excreted through these organs. -Pregnancy and Breastfeeding: The safety of granisetron during pregnancy and breastfeeding has not been fully established. Use with caution and only if the benefits outweigh the risks.


Side Effects

Anxiety, fever, headache, asthenia, somnolence, dizziness, insomnia. Elevation of liver enzymes (AST & ALT).


Drug Interactions

Inducers or inhibitors of hepatic Cytochrome P450 drug metabolising enzymes: Granisetron does not induce or inhibit the cytochrome P450 drug metabolising enzyme system.
